About the Service: 

 

This admission is usually requested by the Carer. The person requiring care would have been assessed by DSL and allocated so many days per year. A form is sent out to the Carer and this is required by us prior to admission. To admit the patient, we must also have a medication chart from their local Doctor stating all the medications they are taking. Bookings are essential as we only have limited Carer Support Beds
